COMMUNITY HEALTH NEEDS ASSESSMENT
Jackson Park Hospital prides itself on working to meet the health related needs of our entire community.
The Affordable Care Act (ACA) of 2010 requires each hospital facility in the U. S. to conduct a Community Health Needs Assessment (CHNA). All hospitals must assess the health of the entire community they serve, not just the patients who walk into their building, and they must make a plan to improve community health. This report brings together health data and the input of our community members to describe the health needs of the community. The data findings and the community input helps us determine the most important local health issues and needs to address as a hospital. The Community Health Needs Assessment and corresponding plan of action helps Jackson Park Hospital continue to improve community health throughout the areas we serve.
The Community Health Needs Assessment is updated every three years and includes participation and input from a wide array of stakeholders in the local community as well as the Metropolitan Chicago area.
